Centerville, Ohio (December 16, 2019) – Bethany Village is pleased to announce a perfect survey for assisted living. This means officials with the Ohio Department of Health found zero deficiencies in our care, services or environment and that Bethany Village has met or exceeded highest levels expected for skilled nursing facilities in the state. The Centers for Medicaid and Medicare Services (CMS) set the standards and regulations.
Facilities in Ohio undergo at least one unannounced survey every nine to 15 months. Achieving a zero-deficiency survey is increasingly difficult and speaks to the amount of teamwork among Bethany Village employees.

This perfect survey comes on the heels of a zero-deficiency survey for the skilled nursing facility at Bethany Village in the fall. This means Bethany Village is a 5-star rated facility with CMS.
Crescent Crossing assisted living opened in January. Over the past 12 months, Bethany employees have welcomed and moved in nearly 90 residents to the new building. Between scheduled activities, meal times, impromptu gatherings and worship services, these residents have become a strong community.
Once construction on Crescent Crossing finished, attention turned to Pavilion, Bethany’s reimagined assisted living-memory care unit. Set to open in late December, Pavilion features two floors of redesigned living spaces, including open concept dining, common areas and an expansive patio for outdoor living in warmer months. Pavilion will offer residents the safety to roam and explore and activities will spark positive memories in those with dementia. Pavilion is comprised of 35 refreshed rooms across two floors. Residents will enjoy family-style dining, game nights and have access to a nurse 24/7.
Bethany Village is located on 100 acres in Centerville, Ohio. The continuing care retirement community (CCRC) is home to more than 700 residents. Each year, an additional 700 individuals choose Bethany for short-term rehabilitation services. Bethany Village is sponsored by Graceworks Lutheran Services, a large non-profit organization dedicated to helping seniors or adults with intellectual disabilities live with dignity and wholeness.
